When an individual is linked with Home and Community Based Waiver Service providers, including residential and vocational services, in DuPage, Kane, and Kendall Counties - and Hanover Township of Cook County, DayOne PACT provides a state mandated advocacy service called Individual Service and Support Advocacy (ISSA).
This service is designed to review the individual's satisfaction with the services they are receiving and provide service coordination for them. We meet with the individual at least four times a year, where they receive services so that we can maintain a meaningful relationship with them and advocate effectively for them.
Services include:
Review and track individual service planning process and service delivery
Attend staffings with individuals receiving services
Visit individuals at residential and day/vocational locations
Review clinical and file documentation
Reconfirm the person's choices and service satisfaction
